Columbus Neighborhood Health Centers, Inc. (CNHC)

Description

The Columbus Neighborhood Health Centers, Inc., are composed of five clinics
through out the greater Columbus area serving an indigent and homeless
population. Services developed since 1991 include practice in the areas of:

  • Asthma Hypertension
  • Dyslipidemia
  • Diabetes Adherence
  • Smoking Cessation
  • Polypharmacy
  • Medication Access Programs

Health Care Providers

Patients are served in a multidisciplinary approach including 2 pharmacy faculty,
physicians, and nurses. This also serves as a ambulatory care pharmacy
residency site and PharmD clerkship site.

Clinical Partners

Description

Clinical Partners is a pharmacist-managed ambulatory clinic located in the outpatient clinic building. Services developed since 1996 include practice in the areas of:

  • Anticoagulation
  • Dyslipidemia
  • Medication Management
  • Diabetes
  • Hepatitis C
  • Smoking Cessation
  • Wellness Herbs and dietary supplements

Health Care Providers

Clinical Partners provides care by a team of pharmacists including 3 pharmacy faculty with a medical director for the Anticoagulation Service and collaboration with physicians, nurse practitioners, and dietitians. This also serves as a community pharmacy residency site and PharmD clerkship site.

University Health Connection (UHC)

Description

University Health Connection is an interprofessional healthcare practice serving the faculty and staff of the university. The primary focus of this practice is health and wellness and connecting employees to the benefits of the health plan. Services developed sine 1999 include practice in the areas of:

  • Urgent Care
  • Medication management
  • Longitudinal Care
  • Wellness and Prevention
  • Work Health
  • Disease Management
  • Limited prescription services

The clinic provides walk-in urgent care, appointments for longitudinal care and disease management, and various screenings. Patient experience an interprofessional team including alternative therapies and pharmacy services in one location.

UHC provides care in an interprofessional approach including physicians, nurses, pharmacists including 1 pharmacy faculty, and alternative care providers. This also serves as a community pharmacy residency site and PharmD clerkship site.

Kroger Patient Care Center (KPCC)

The Kroger Patient Care Center serves as a resource center for patient and pharmacist education in the Great Lakes division of the Kroger Company. Services developed since 1999 include:

  • Wellness screenings
  • Medication Reviews
  • Diabetes management
  • Drug information services
  • Dyslipidemia management
  • Smoking cessation
  • Immunizations
  • Community outreach and education

The Kroger Patient Care Center offers care by 1 pharmacy faculty and a team of pharmacists. This also serves as a community pharmacy residency site and PharmD clerkship site.

Cardinal Health Visiting Pharmacist Program

Description

The Cardinal Health Visiting Pharmacist Program brings medication assessment to home bound individuals who might not otherwise have access to such a review. The referral resource is LifeCare Alliance, the agency in Central Ohio who provides the Meals-On-Wheels program and other home based services. The interventions commonly made by performing an assessment vary but include the following:

  • Compile a typed medication record Review for potential side effects
  • Review for interactions/duplications Suggest ways to simplify regimen
  • Give OTC drug information Look for medication cost resources
  • Clarify drug coverage Examine safe medication practices

Health Care Providers

The program is offered by one part time clinical pharmacist who interacts with the visiting nurses and wellness programs of LifeCare Alliance. The clients’ other health care providers are also contacted via phone or letter on a case by case basis. This is also a PharmD clerkship site.

Senior Low Income Medication Program

Description

Since 2002, this service has been provided in the homes of people over 62 years old living in HUD subsidized apartments. Services include:

  • Medication Evaluations Education on devices
  • Medication compliance Education on chronic diseases
  • Medication Assistance Programs Healthcare consumer education

Health Care Providers

One pharmacist (faculty), Year III and Year IV students provide services in cooperation with Catholic Social Services and funding from the Merck Foundation. The goal of this program is to ensure that patients have access to prescription medications and are more active healthcare consumers.

HIV High-Risk Pregnancy Clinic

Description

The multidisciplinary HIV High-Risk Pregnancy Program provides seamless
care to economically disadvantaged and underserved minority women
infected with HIV. Services provided include:

  • Health and medication education
  • Chemical dependency counseling
  • Funding support from Ryan White Title Four Grant

Health Care Providers

The personnel support at the HHRP include medical specialists in Infectious
Diseases, Obstetrics and Gynecology and Pediatrics, nurses, pharmacists,
social workers, dietitians, case managers, interpreters, outreach coordinator, and
local and state health agencies. This is also a PharmD clerkship site.
