Practice

Pediatric Blood and Marrow Transplantation Program

I provide clinical pharmacy specialist services at Nationwide Children’s Hospital, Columbus, OH to the Pediatric Blood and Marrow Transplantation Program through the Department of Pharmacy.  For these services the College of Pharmacy, The Ohio State University is reimbursed for 50% of my salary and benefits by Nationwide Children’s Hospital, Dept. of Pharmacy making me a 0.5 FTE. The clinical services provided by me include but are not limited to the following:

  1. As part of a multi-disciplinary health care team consisting of a BMT/PSCT attending physician, a Hematology/Oncology medical fellow, a BMT/PSCT nurse practitioner and a social worker I provide medication therapy management during daily patient care rounds
  2. I am available for medication therapy management of BMT/PSCT patients via pager 7 days/week, 24 hours/day for BMT/PSCT patients except vacation and illness.
  3.  As needed I also provide medication therapy management to the BMT/PSCT Outpatient Clinic.  This service is provided as needed since these patients are seen simultaneously as the inpatients by a separate team.

The functions performed by me under medication therapy management include but are not limited to:

  • Collecting and organizing patient data such as demographics, drug profiles, physical findings and relevant laboratory values;
  • Providing information on drug doses, drug dosing adjustments during renal and hepatic impairment, adverse effects to be anticipated, drug-drug interactions, commercial dosage forms available, and extemporaneous dosage form availability.  This is an important service provided especially regarding drugs not commonly used in or well studied in pediatric patients;
  • Providing pharmacokinetic consultation for drugs such as aminoglycosides, vancomycin, cyclosporine, tacrolimus, antiepileptics such as phenytoin, and phenobarbital, and chemotherapy agents such as busulfan;
  • Providing preliminary recommendations (preceding Nutrition Support Consult Team recommendations) on total parenteral nutrition in BMT/PSCT, recommending total calories, amount of carbohydrates, protein, and lipids, volume of fluid and electrolyte composition of the TPN;
  • Providing recommendations on fluid and electrolyte supplementation (boluses and through IVF) and changes to oral supplements during discharge planning as well as when seen in the outpatient clinic
  • Providing consultation on drug therapy use for specific patient care issues such as antiemetics, chemotherapy adverse effects, pain mangament, veno-occulsive disease, and graft versus host disease.
  • Checking and approving chemotherapy and supportive care drug orders for accuracy prior to patient admission for a myeloablative chemotherapy preparative regimen prior to BMT/PSCT
