Graduate Admission Application Checklist for All Applicants to the PhD Program
- Completed Application Form :
The application deadline for autumn quarter admission is January 1st .
If you cannot complete an application on line at: http://gradapply.osu.edu, please use the following instructions for completing the paper application:
1. Intended Graduate Program and Program-related Information . Write " PHARMACY " in the
block labeled "Intended Graduate Program/Department" and " 360 " in the "Program Code" block.
2. Intended Area of Specialization . Specify Pharmaceutical Administration in the box labeled "Intended Area of Specialization".
3. Financial Aid . To apply for financial aid, check the appropriate boxes on the application marked "Financial Aid" . This application form serves as an application for a graduate stipend appointment for domestic and international applicants. No other form is required. Send the completed application with the application fee ($30/domestic and $40/international) to: Admissions Office, The Ohio State University, 3rd Floor Lincoln Tower, 1800 Cannon Drive, Columbus, OH 43210-1291.
- Official Transcripts : Two sets of official transcripts are required from each college/university attended. Please have these sent to the Admissions Office. Photocopies are not acceptable . The minimum GPA requirement is 3.0 for undergraduate coursework, and 3.0 for previous graduate coursework. If any of your cumulative grade point averages from any of your previous institutions or degree programs fall below 3.0, you should call me immediately to discuss (Craig A. - Autobiography : Write a one-page autobiography and mail directly to: The Office of Graduate Studies and Research, OSU College of Pharmacy, 217 Lloyd M. Parks Hall, 500 West 12th Avenue, Columbus, OH, 43210-1291 . The autobiography should tell us about yourself and how this graduate training program will help you meet your career goals. Please include your email address in your autobiography .
- Letters of Recommendation : Ask three individuals (preferably faculty) to complete the 2-page "OSU Graduate School Personal Reference Form". This form is available at the OSU Graduate School website (http://www-afa.adm.ohio-state.edu/apps/pdfs/refer.pdf). Each recommender must complete the 2-page form and provide a personalized letter of recommendation. Attached letters must be typed on official, original, letterhead paper with a return email address, telephone of FAX number. (Applications with recommendation letters that do not meet this requirement will be automatically denied ). Recommendations should be mail directly to the College address listed in the previous paragraph.
- Official GRE scores are required of all (international and domestic) applicants . Please visit http://www.gre.org/splash.html to register for the exam and get the most recent information. Total minimum score requirement is: 1100 (verbal and quantitative section); however, higher scores are preferred. On the new GRE, the analytical score requirement is 3.0. Please note: GRE "subject" scores are not required. When completing the GRE application ( use the following codes: The institution code for OSU is "1592". The Department Code for pharmaceutical sciences is "0613".
- International Applicants (Not U.S. Citizens) : Please refer to the instructions that follow for additional requirements.

- English Proficiency : All international applicants are required to submit the results of either the Test of English as a International Language (TOEFL) or the Michigan English Language Assessment Battery (MELAB). Have official scores of either of these tests sent to the Graduate School Admissions Office and a copy to: The Office of Graduate Studies and Research, OSU College of Pharmacy, 217 Lloyd M. Parks Hall, 500 West 12th Avenue, Columbus, OH 43210.
- TOEFL Requirement : A minimum TOEFL score of 600 (250 computer-based) is required for admission to pharmaceutical administration. If your score is below the requirement, your application will be automatically denied. Official scores are required. Please note: (The institution code for OSU is "1592". The two-digit code for pharmacy is "47").
- Spoken English Proficiency : All international applicants who want to be considered for Graduate Teaching Associateships (GTA's) are required to take the Test of Spoken English (TSE). Please have the official test scores sent directly to the Admissions Office. If you are unable to take the TSE before arriving at the University, our Spoken English Department offers a teaching exam at the beginning of each quarter.
- Applicants with a master's degree have a better chance for admission.
