Kroger Patient Care Center Outreach Program

A partnership between the Kroger Company and The College formed the Kroger Patient Care Center (KPCC). This partnership serves the community by providing an expanded level of pharmaceutical care to the community. This care includes wellness and screening services such as glucose screenings for diabetes, cholesterol screening for hyperlipidemia, and bone mineral density screening for osteoporosis. In addition, disease state management programs provide pharmacists with the opportunity to work with other health care professionals to help patients better manage their chronic diseases. These services are provided by a College Faculty member, a Kroger pharmacist a community pharmacy resident and experiential students from the College.

Community programming includes:

• Diabetes Education and Nutrition Tours
  
• Heart Healthy Grocery Store Tours
  
• Health Information days for specific diseases and populations
  
• Provide general medication info at area health fairs and events such as the annual Women's Show
