To apply online or to print an application, please access the Admissions Office web site at http://gradadmissions.osu.edu and follow these steps:

1) Apply On-Line Instantly
2) Click on "All Other Graduate Programs"
3) Click on "Create Your Account"
4) Fill in your information
5) Click on "Graduate Applications" and "I Agree" (reference forms are available on this page)
6) "Online Graduate Application"
7) Sign in on the fourth page
8) Your specialization can be abbreviated

Tuition

For 2008-09, full time, Graduate School fees for Ohio residents are $3,561/quarter and $8,641/quarter for non-residents.

Financial Aid

Current stipends for Graduate Research Associates, Graduate Teaching Associates and Graduate Fellows are $22,000 to $27,000/year. Tuition is totally waived. To apply for financial aid, mark the appropriate boxes on the application. No other form is required. If you are printing the application, please send it with the application fee ($40 domestic; $50 international) directly to the Admission Office, The Ohio State University, 3rd Floor Lincoln Tower, 1800 Cannon Drive, Columbus, OH 43210-1291.

Official Transcripts

Two sets of official transcripts are required for each college or university attended. One set should be sent directly to the Admission Office, and the second set should be sent to our graduate program office. Photocopies are not acceptable. The minimum gpa requirement is 3.0 for undergraduate and graduate coursework.

Statement of Purpose

Write a two-page Statement of Purpose and mail directly to:

Letters of Recommendation

Please ask three individuals (preferably faculty) to complete the “OSU Graduate School Personal Reference Form” and complete the recommendation letter on line.

GRE Scores

Official GRE scores are required as follows: a minimum score 3.0 on the written portion and the combination of verbal and quantitative should be at least 1103 on the IBT. GRE "Subject" scores are not required. OSU Institution code is "1592"; Department Code for pharmaceutical sciences is "0613". The Education Testing Service should send your score report directly to our admissions office. Please send a copy to our office also."

Additional Requirements for International Applicants

English Proficiency: All international applicants are required to submit the results of the Test of English as a Foreign Language (TOEFL). Official scores should be sent to the Admissions Office, and a copy should be sent to The Office of Graduate Studies and Research.

TOEFL Requirement: A minimum TOEFL score of 100 (IBT) is required. If your score is below the requirement, your application will not be considered for admission.

Please Note: The Institution Code for OSU is “1592” and the two-digit code for Pharmacy is “47”

Health-System Pharmacy Administration

The information provided in this paragraph applies to applicants to the Health-System Pharmacy Administration Program (formerly Hospital Pharmacy) ONLY. Applicants to this program must have a degree from an A.C.P.E. accredited college of pharmacy and be eligible for pharmacy licensure in the United States. Students in this program are required to serve in a residency program in one of four area hospitals. If you are not eligible for pharmacy licensure in the U.S., your application cannot be considered for admission to the Health-System Pharmacy Administration Program.
