MS in Health-System Pharmacy Administration with Combined Residency

Thank you for your interest in the MS in Health-System Pharmacy Administration Residency Program. This page provides instructions on how to apply to the program along with links to applications websites and materials.

Applicants to this program must have a degree from an A.C.P.E. accredited college of pharmacy and be eligible for pharmacy licensure in the United States. Students in this program are required to serve in a residency program in one of four area hospitals. If you are not eligible for pharmacy licensure in the U.S., your application cannot be considered for admission to the Health-System Pharmacy Administration Program.

This is a combined program with a residency, seperate applciations must be made.

Without visiting each hospital, it is difficult for a student to guess where they might find the best residency site for them to develop their skills.  To optimize your chances of finding the best program match, please evaluate and apply to as many sites as possible.  It is possible to interview at all 5 sites in two and a half days.

How to Apply to the Program

To apply to the residency program please complete the following steps:

Step 1) Apply using the College of Pharmacy's application form

Use the following web-form to create an application for the college:

Step 2) Complete the OSU Graduate School Application:

You can complete an application online at the Graduate Admissions website.

The application deadline for autumn quarter admission is in January.

You can check you application status at the OSU Admission Application Status site.

 

Step 3) Prepare your statement of interest for the application.

Write a one-page statement of interest and upload it (in Microsoft Word format)
using the web-form in Step 1.

The autobiography should tell us about yourself and how this graduate training program will help you meet your career goals. Please include your email address in your autobiography.

 

Step 4) Sign up for the Graduate Record Examination (GRE):

Please visit the GRE website to register for the exam and get the most recent information.

The GRE exam format changed October 2002. Please visit http://www.gre.org/splash.html to register for the exam and get the most recent information. In the College of Pharmacy we have minimum scores required by our Graduate Studies Committee. See the attached sheet for required scores. As with the GPA requirements, if you fall below the minimum scores, we must petition on your behalf. Therefore, please take the necessary time to familiarize yourself the GRE exam format and the information tested prior to taking the exam. This should help you to get GRE scores that most closely match your abilities.

 

Step 5) Send two copies of transcripts:

Send two copies of transcripts from ALL schools attended (e.g., pre-pharmacy, pharmacy, community college, previous academic degrees) to:

Official Transcripts : Two sets of official transcripts are required from each college/university attended. Please have these sent to the Admissions Office. Photocopies are not acceptable . The minimum GPA requirement is 3.0 for undergraduate coursework, and 3.0 for previous graduate coursework. If any of your cumulative grade point averages from any of your previous institutions or degree programs fall below 3.0, you should call me immediately to discuss (Cynthia Carnes, PhD 614-292-1715).

 

Step 6) Send letters of recommendation and the two-page graduate application reference form:

Download the Graduate Application Reference Form (PDF) from the OSU Graduate School website.

Ask three individuals (preferably faculty) to complete the 2-page OSU Graduate School Personal Reference Form. Each recommender must complete the 2-page form and provide a personalized letter of recommendation. Attached letters must be typed on official, original, letterhead paper with a return email address, telephone or FAX number. (Applications with recommendation letters that do not meet this requirement will be automatically denied ). Recommendations should be mailed to:
