The APhA Foundation Patient Self-Management: Diabetes program was offered as a pilot project involving 70 OSU faculty and staff. Final aggregate results from this pilot year of the have shown a reduction of hemoglobin A1c values from a baseline of 7.9% to 7.1% . The Table below illustrates the improvements in other program outcomes including lipid profiles, eye and foot exams, and influenza vaccinations. Over 90% of patients reported being satisfied or very satisfied with their pharmacist care.
