Study results comparing our service to inpatient care at OSU Medical Center showed that the Clinical Partners managed service decreased the number of hospital readmissions for anticoagulation related conditions in the first 90 days after initiation of therapy (See Table below). This study compared patients referred to the Clinical Partners Anticoagulation Management Service with matched patients who were treated with conventional physician practice.
Type of Warfarin-related Readmission |

a. Number of cases = one case for each subject with no warfarin-related hospital readmissions during the study period plus the number of warfarin-related readmissions for the remaining subjects (two subjects in the control group experienced more than one warfarin-related readmission during the study period)
Adapted from: Lee YP, Schommer JC. Effect of a pharmacist-managed anticoagulation clinic on warfarin-related hospital readmissions. AJHP 1996; 53:1580-3.
Annual analyses of clinical outcomes from 2002-2010 illustrate that our INR compliance rates are consistently at or above the INR compliance rate of 45% documented in the literature for conventional practice.
In 2003, pharmacists conducted a patient satisfaction survey using a validated tool developed by Dr. David Nau at the University of Michigan. Results of the survey demonstrated that based on a Likert scale of 1 to 5 (1= strongly agree, 5= strongly disagree), patients felt that talking with a pharmacist about how to take warfarin was worth the time (Mean 1.4, 95% CI 1.17-1.64), felt that meeting with a pharmacist regularly decreased their chances of having a recurrence of a thromboembolic event (Mean 1.6, 95% CI 1.26-1.87), and 100% of patients stated that they would refer friends and family to the Clinical Partners AMS.
